General Info

On your first appointment you will be filling out one page Health History form.

Before your first appointment:

• Make sure to know where my location is to be on time.
• If you can not make it for your appointment please give us a 24 hour notice. There is a 100% fee for missed appointments. It will not be applied in cases of emergency.
• Try not to schedule anything stressful or strenuous immediately after your appointment, so your body can process the healing without interruption.

You will be charged for missed appointments or late cancellations at the full appointment cost.
Emergencies are exempted.
